The Department of Rheumatology of Hygeia Hospital is organized to offer a complete diagnostic and treatment service for the rheumatologic diseases through:

The ambulatory service, where all the patients that come for consultations with a rheumatology specialist get their appointment.

Daily service, which is reserved mostly for the patients that do not need prolonged stays in the hospital, but need to undergo medical treatments that have a necessary dynamic follow-up during some hours from medical and nursing staff, eg, intravenous administration of drugs (biologic medications, medications for osteoporosis, etc).

Hospitalized service, with a ward and beds that are used by the patients that need to undergo a more specific evaluation and treatment course, until their health status is stable. The physicians of the Department of Rheumatology of Hygeia Hospital, besides their service in this department, are engaged also in the diagnosis and treatment of all the patients that, besides their primary disease, have also rheumatic complains.

The management of the patients in the Department of Rheumatology of Hygeia Hospital is done utilizing the data from the complete clinical-laboratoric evaluation of the patient, which makes possible to choose the right scheme of treatment. Before starting the treatment plan, the specialists discusses with the patient the scheme, explaining carefully and in a comprehensive way the application of the medications to achieve the desired results. To treat the rheumatologic diseases in our department we use monotherapies or combined therapies (pluritherapies) according to the stage of disease and aiming always the improvement of the  patients complains. Amongst the most important drugs we use to treat the rheumatoid diseases are:

Analgesics, which help to ease the pain and also to increase the effect of other medications in combined therapies.

Nonsteroid antiinflamatory medications, utilized to reduce the pain and swallowing. The nonsteroid antiinflamatory effect  starts almost immediately after the administration and lasts for some hours, that is why we recommend to the patients to follow the scheduled administration. Amongst the NSAIDs we can mention: ibuprofen, diclofenac, indomethacin, nimesulide, naproxen, etc.

Steroid anti-inflammatory medications, glucocorticoids, reduce impressively the inflammation. Even though they don’t cure the disease, they are able to slow the evolution of the disease, improving the clinical symptoms of the patients with rheumatismal disease. These medications exist in the form of tablets, capsules, which can be administered by oral route, or they can be in a fluid form, for intravenous, intramuscular or intraarticular use. The medications of this group are, prednisone, prednisolone, methylprednisolone, dexamethasone, etc.

DMARDs are drugs that modify rheumatoid disease and “fight” the disease and not the symptoms. These medications do not act immediately as NSAIDs, but need some time to achieve their full effect. These drugs are classified as: traditional and biologic. In the traditional class the most utilized are Methotrexate, Hydroxychloroquine and Sulfasalasine, while in use are also a subcategory, known as immunosupresants, as, Azathioprine, Leflunomide, Cyclosporine. These drugs slow down the exaggerated activity of the immune system and are utilized for some time always in close follow-up from the specialist of the disease activity. In this way it is possible to minimize gradually the dose in correlation with the disease activity. In the cases when the traditional modifying drugs do not give the expected effect, we start the biologic medications, which target the molecules and/or special proteins involved in inflammation and tissue damage. These medications are increasingly utilized the last 10-15 years. Among the most utilized are: Adalimumab, Etanercept, Infliximab, Rituximab, Tocilizumab, Abatacept, Certolizumab etc.
Against osteoporosis we prescribe: calcium and Vitamine D, Bisphosphonate (Alendronate, Risedonate, Ibandronate, etc), teriparatidet, raloxifen, denosumab, hormonal replacing therapy. Etc.

All these drugs have side effects, that is why we recommend their careful use. These drugs are administered after the complete evaluation of the health status of the patient from the specialist. For example, the doctor can add calcium and vitamin D in the patients that will receive glucocorticoids in order to minimize their effect in the osseous density, or, they can add a protector of gastric mucosa in the case of the use of NSAIDs, etc. The medical management of the patients in the Department of rheumatology of Hygeia Hospital Tirana is based not only in the use of drugs, but also in a lot of other elements that are necessary to improve the health status of our patients. In this aspect, an important role plays also the patient’s information on the disease. We explain carefully the main disease, the factors that can worsen or improve it; we chose the right drugs, and explain their use and the possible side effects; we explain the importance of an equilibrated diet, the interruption of smoking, which is accused as one of the main factors responsible for the rheumatologic diseases. All these steps aim to help the patient to learn to live with the disease minimizing its effects in everyday life. The continuous monitoring of the patients make possible to control not only the side effect of therapy, but also the benefits from the scheme of treatment. The department of Rheumatology of Hygeia Hospital saves all the data in electronic systems in order to have a continuous and periodic evaluation of the disease.

Also, in our department we offer the service of Physiotherapy to rehabilitate the patients through the right physical exercise that can improve the symptoms, the range of motion, and also increase the bone stability and the muscular tone.

To manage the complications, our department of rheumatology collaborates also with the other departments of our hospital, like Dermatology,neurology, nephrology, infectious disease, orthopedics, etc.

The Department of rheumatology of Hygeia Hospital Tirana, in order to improve their service, participates in different scientific programs and meetings in the country and abroad, bringing the latest and contemporaneous treatment schemes for the patients.
